Mental Health Clinician

Job description
- Conduct outreach and build trusting relationships with individuals in the community
- Provide short-term counseling, crisis intervention, and supportive services
- Complete psychosocial and risk assessments and develop individualized care plans
- Support participants before, during, and after transitioning into housing
- Connect individuals with healthcare, housing, and community resources
- Coordinate with hospitals, outreach organizations, social services, and other community partners
- Help participants navigate appointments and overcome barriers to care
- Support case managers and peer staff as part of a multidisciplinary team
- Document progress and contribute to team meetings and care planning
- Master’s degree or higher in social work, mental health counseling, psychology, nursing, or a related field
- Appropriate professional licensure required
- Experience with individuals experiencing homelessness, mental health conditions, or substance use disorders is a plus
- Strong counseling, crisis management, de-escalation, and community outreach skills
- Person-centered, trauma-informed, and harm-reduction approach
- Strong communication, documentation, and collaboration skills
- Comfortable working in the field with flexible hours and occasional on-call coverage
Schedule: Sunday-Thursday 9AM-5PM
