Scismic is supporting the growth of a non-profit corporation that has been facilitating a wide range of support to people with mental illness for more than 45 years. They provide outreach and engagement, care coordination, peer support, supportive housing and rehabilitation to adults with serious mental illness, and promotes access to treatment, employment, and other supports and services according to an individualized plan that addresses a client’s strengths, needs, goals, and choices.

Position Summary

We are looking for a Team Leader to build an interdisciplinary team, lead with a startup mindset, and create new frameworks for their team to serve vulnerable New Yorkers. Join us as Team Leader, Critical Time Intervention (CTI), where you'll oversee the operations and services of a new and dynamic program, ensuring it delivers life-changing results with precision and care.

 

Key Responsibilities:

  • Lead the delivery of CTI program services with fidelity to the CTI model and compliance with OMH and Medicaid standards.
  • Build, develop, and lead a team: Recruit, supervise, and train program staff.
  • Develop and implement safety and emergency frameworks to ensure seamless operations.
  • Partner with hospital systems to manage referrals and facilitate access to services.
  • Oversee outreach and engagement, admissions and discharges, assessment, case presentations and team assignments.
  • Conduct quality assurance audits and activities to assess the effectiveness of services delivered, to ensure member charts & documentation are in compliance, and to ensure that quality services are being delivered to clients.
  • Certify the eligibility for billing to third parties for program services rendered.
  • Drive team effectiveness through weekly reviews, caseload management, and follow-ups.
  • Coordinate staff schedules and manage on-call requirements ensuring timely response to referrals and clinical events
 

Who You Are:

  • A Licensed Practitioner of the Healing Arts (LPHA) with 3–5 years of post-licensure experience working with individuals facing severe mental illness or co-occurring disorders.
  • Experienced in collaborating with hospital systems and familiar with clinical documentation, motivational interviewing, and stage-wise treatment.
  • An experienced supervisor with excellent written, verbal communication, and organizational skills.
